Terms of Service

1. Introduction

I. Arataki Wellness Center, provides general and specific personalised services supporting patients requiring access to information/resources , health and wellness services.
II. Where medications are prescribed as ‘unproven’, or ‘ off label’, these medications are provided in compliance with NZ laws and regulations such as Section 29 of the Medicines Act 1989.
III. Arataki Wellness Center, provides a specialised service only and cannot be a substitute of your regular Family doctor (GP). IV. Arataki Wellness Center, conducts its services by using Telehealth modalities. Telehealth includes those services conducted via teleconferencing (audio/video), secured email, and text messaging.
V. All medical advice is provided by or under the supervision of a Registered Medical Practitioner with the NZMC. 

2. User guidelines (rules, restrictions, requirements)
I. A service user requiring a specific personalised service must register with the clinic before any services can be provided.
II. If a service is requested by some one other than the patient her/him self, an EPOA is required.
III.Only New Zealand based residents are eligible to access our services. Only those persons that are physically present in New Zealand at the time of their request/evaluation are eligible to access our services. 
IV.Users who submit incorrect, inadequate ( no dosing instructions, total amount of supply ) or non-verifiable information including personal details, GP details will not be provided with a prescription. Furthermore, such submissions are NOT Eligible for refund.
V.Submissions requesting medicines with specific Brand Names that are not available on the New Zealand Medicines Schedule , will be replaced with a similar generic medicine that is available in New Zealand.
VI.AWC will not provide prescriptions for the purpose of obtaining medicines from outside of New Zealand.


3. Privacy & Health Information
I. The service user agrees to provide correct information.
II. The service user agrees that AWC can send relevant health information to the clients nominated family doctor (GP).
III. The service user agrees that the clinic will share their patient information as required with those government agencies and professional bodies that oversee the compliance with legislative and professional standards.

 4. Copyright and intellectual property
 All information on our website, electronic devices, print and electronic format are property of the clinic.

 6. Termination of accounts/service
 Arataki Wellness Center, has the right to decline/terminate any services at any time.  

7. Complaints
All complaints can be directly sent to Arataki Wellness Center. You can call us directly on 027-330-0521. It is your Right to lodge a complaint with the Health and Disability Commissioner on 0800 112233.

8. Payment terms
 All services are pre-paid via electronic payment platform on Paypal.
This accepts Debit/Credit Cards (Visa, Mastercard, AMEX) options.

Wise*** provides payment transfer account services supervised by NZ Department of Internal Affairs, and for a nominal one-time $14NZD card fee provides you instant access to a Debit Card usable on Paypal.

 9. Refunds
 Refunds are only issued on a case-by-case basis at the discretion of AWC.

10.For General Practice Only
Practice Contracting Online-Prescription Service Agreement (PCOLPSA)
Arataki Wellness Center Limited (AWC) provides online medical prescriptions either directly to the patient or at the request of the patients general practice (GP).
1. The requesting GP understands and agrees to share all relevant patient medical information to enable an appropriate patient evaluation and suitability for issuing a prescription.
2. The requesting GP understands and agrees that for the purposes of this service patient privacy and confidentiality agreements already in force include AWC Ltd personnel.
3. The requesting GP will provide all required patient information in accordance with AWC published protocols.
4. The requesting GP will pay AWC within 7 working days after receiving Invoice for services rendered.
5. The individual who is submitting the ‘Request for Prescription’ has the authority (delegated or otherwise) to do so.
6. AWC agrees to provide Online -Prescription service to the Requesting GP
7. AWC undertakes not to enrol any referred patients to its services
8. AWC guarantees not to claim any ‘claw back’ funding whilst providing this service

 By submitting this Request for Prescription You agree to the Terms of Service Agreement as published online.

11.For Pharmacy Only
Practice Contracting Online-Prescription Service Agreement (PCOLPSA)

Arataki Wellness Center Limited (AWC) provides online medical prescriptions either directly to the patient or at the request of the pharmacy servicing the patient. 

1. The requesting Pharmacy understands and agrees to share all relevant patient medical information to enable an appropriate patient evaluation and suitability for issuing a prescription.
2. The requesting Pharmacy understands and agrees that for the purposes of this service patient privacy and confidentiality agreements already in force include AWC Ltd personnel.
3. The requesting Pharmacy will provide all required patient information in accordance with AWC published protocols.
4. The requesting Pharmacy will pay AWC within 7 working days after receiving Invoice for services rendered.
5. The individual who is submitting the ‘Request for Prescription’ has the authority (delegated or otherwise) to do so.
6. AWC agrees to provide Online -Prescription service to the Requesting Pharmacy
7. AWC undertakes not to enrol any referred patients to its services
8. AWC guarantees not to claim any ‘claw back’ funding whilst providing this service

 By submitting this Request for Prescription You agree to the Terms of Service Agreement as published online. 

12 . Pricing
Prescriptions
Prescription 1st encounter $30
Second encounter with NO changes* $10
Repeat prescription with changes ** $25
Urgent option - within 24 hours - additional $11.00


Second opinion/ referrals by booked appointment VC/AC $150.00 for 20 minutes

Top up Payments - linked here

*No Changes - means, the request prescription is exactly the same as the previous one prescribed by AWC. (Caution - be aware if your medications have been changed since our last encounter).
**Changes includes: change of dose, change of brand, change of nominated pharmacy/chemist, added medication(s), dropped medication(s)
